Dr. Bain does not accept insurance or submit insurance claims.  Many of the services he provides may be covered by your insurance under   “out-of-network” benefits. Consult your benefits administrator for details of coverage under your policy.

Payment by check or credit card (AMEX / Discover / Mastercard / VISA) is required at the first visit.  You will receive a monthly statement of your account showing all charges and payments to date.  Payment is expected upon receipt of the statement.

Appointment time is reserved:  there will be a fee charged for any appointment not cancelled at least 24 hours in advance.
